\u003cp\u003eTelecare\u0026rsquo;s Community Alternatives Team (T-CAT) provides the following outreach services in Pierce County:\u0026nbsp; The\u0026nbsp;Diversion Service\u0026nbsp;works in partnership with Community Hospital Emergency Departments and Pierce Co. Mobile Crisis Team to assess risk and provide additional support to individuals who can safely avoid a psychiatric hospitalization with appropriate support and resource referrals.\u0026nbsp; The\u0026nbsp;Transition Service\u0026nbsp;serves individuals who are working towards discharge from an E\u0026amp;T or other housing setting and who could use additional support in successfully sustaining their transition to outpatient services (i.e. interim prescribing support; connecting with outpatient mental health services; seeking housing, employment or education opportunities; connecting with others in social settings, and other supportive services).\u0026nbsp;The Transition Service also receives referrals from many other community stakeholders to address an individual\u0026rsquo;s emerging needs in the community such that a return to an acute hospital setting can be prevented wherever safely possible.\u003cstrong\u003e\u003cspan\u003e\u0026nbsp;\u003c/span\u003e\u003c/strong\u003e\u003c/p\u003e

This involves person centered recovery planning and collaborating with other services and agencies.\u003c/p\u003e
\u003cp\u003e\u003cstrong\u003eESSENTIAL FUNCTIONS\u003c/strong\u003e\u003cbr /\u003e\u0026bull; Demonstrates the Telecare mission, purpose, values, and beliefs in everyday language and contact with internal and external stakeholders\u003cbr /\u003e\u0026bull; Provides safe, effective, and efficient implementation of direct care in accordance with established policies, procedures, and standards of care\u003cbr /\u003e\u0026bull; Works with members served and multidisciplinary treatment teams to design, implement, and evaluate the Recovery Plan \u003cbr /\u003e\u0026bull; Under clinical supervision, establishes and maintains a therapeutic relationship with members served\u003cbr /\u003e\u0026bull; Ensures provision of needed clinical services to assigned members served; coordinates ongoing treatment within the program and with outside agencies\u003cbr /\u003e\u0026bull; Provides individual and group therapy\u003cbr /\u003e\u0026bull; Documents all treatment provided in a thorough and consistent manner \u003cbr /\u003e\u0026bull; Collaborates with other staff in the discharge planning process and makes linkages to community services for members served\u003cbr /\u003e\u0026bull; Actively participates in multidisciplinary team meetings and recovery planning meetings\u003cbr /\u003e\u0026bull; Demonstrates knowledge of de-escalation techniques and crisis communication and management\u003cbr /\u003e\u0026bull; Serves as a clinical consultant to recovery team members\u003cbr /\u003e\u0026bull; Must evade members served in the event of assaultive behavior and pass assault crisis/crisis prevention training\u003cbr /\u003eDuties and responsibilities may be added, deleted, and/or changed at the discretion of management.\u003c/p\u003e
