In May 2015, TriStar Horizon
opened TriStar Natchez ER, an 8-bed full service ER located at the
TriStar Natchez Medical Park on Highway 46. In addition to the existing 26-bed
emergency room at TriStar Horizon, this full service ER provides emergency care
as well as imaging services and comprehensive laboratory testing.

 

TriStar Horizon Medical Center is looking for a Master Social Worker (MSW) or Licensed Clinical Social Worker (LCSW).This is a Full-Time, Exempt position.POSITION SUMMARY

The
MSW or LCSW provides direct social work services that are based on the treatment plan’s
established priorities through analysis of psychosocial assessment, counseling
and community service coordination. Assists with discharge planning to
facilitate continuity of care and the transition of patient with personal or
environmental difficulties which may interfere with obtaining maximum benefit
from medical care. Facilitates interdisciplinary team approach to meeting the
broad spectrum of the patient’s healthcare needs.  Appropriate interpersonal and communication
skills. Evidence of analytical and decision-making skills. Participates in Performance
Improvement activities to assure continuous Quality Improvement. Maintains
cultural and diversity awareness when dealing with patients, visitors,
co-workers and other customers. Promotes a supportive, team-work oriented
environment. Fulfills departmental responsibilities with the long-range plan,
mission, vision and values of Horizon.

','!*!Master’s
degree in Social Work required. Two years experience in mental health field
and/or acute care environment required. Computer skills preferred. Ability to
take evening and weekend call. Basic Healthcare Provider (BLS) Certification
required within 90 days of hire or promotion.
