We are looking for a Clinical Coordinator for our PROS Mental Health program in Brentwood, NY. This dynamic individual would provide clinical services, review case records to ensure compliance with agency and regulatory guidelines, provide ongoing clinical supervision and function as an integral member of the multidisciplinary team.
function as a primary counselor for an assigned caseload of 10 adults with
co-occurring disorders, providing assessment, individual counseling, crisis
intervention, individualized recovery planning and discharge planning
â€¢ To facilitate groups which address
client rehabilitation and recovery needs
maintain timely documentation in compliance with agency and regulatory
â€¢ To provide bi-weekly
clinical supervision to all direct care staff and submit accompanying
documentation to the PROS FIRST Program Director
â€¢ To review case records of new admissions and assign
review case records of all discharged clients, to ensure compliance with
agency and regulatory guidelines
review 25% of all active case records for compliance and quality of content,
and to complete the accompanying PROS Case Record Documentation Review Form
â€¢ To complete monthly tracking sheet for
â€œminimum billing requirements for case record compliance
â€¢ To submit monthly PROS compliance
report to Compliance/Quality Assurance Director
â€¢ To function as an integral member of a multidisciplinary
â€¢To assist the PROS Program Director
with program development
provide clinical staff with ongoing consultation
â€¢To oversee all client case conferences
â€¢ To perform any other related duties and functions, as assigned by
the PROS FIRST Program Director
The schedule is Monday through Friday, 9:00 AM to 6:00 PM. The position requires 20% travel.
Masters Degree in Human Services or related field, from an accredited college or university
LCSW or LMHC in good standing in NY state
Minimum of three years providing individual and group treatment service delivery to mental health and substance abuse population
Minimum of one year providing clinical supervision
Experience documenting in an Electronic Health Record system
Crisis De-escalation skills
Ability to problem solve and troubleshoot
Excellent writing and communication skills
