Profile

Job Description Promote and facilitate community linkages with clinical resources and social resources for clients in the Staunton, Augusta, and Waynesboro communities. Works to compliment the activity of the Public Health Nurse and providers to help manage care of clients. Focuses less on clinical care, and more on navigating the healthcare system through referral support, assistance with appointments, and education on appropariate use of resources. Has strong relationships with community partners, and knowledge of available services. Networks with partners to ensure a safe, holistic, and complete care plan is achieved. Provides thorough follow up as needed. Must have a good understanding of the healthcare system, social determinants of health, and have demonstrated good communication skills verbally and in writing. This position serves as a core member of the home visiting team and must be able to provide health education independently. Minimum Qualifications A minimum of High School education or GED. Preferred Qualifications Fluent in speaking and writing in English and Spanish. Previous public health experience. Experience with health promotion and/or health education. Knowledge of the local community. Knowledge of Maternal-Child Health. Relevant healthcare experience. Special Requirements It is the practice of the Virginia Dept of Health to conduct in-depth background checks on all candidates identified as a finalist for employment consideration. Must have or be eligible to obtain a VA drivers license and provide own transportation (mileage reimbursement provided). Employee may be designated as essential personnel during emergency events outside of normal work hours. Twelve-month probationary period is required for all new state employees.
