ESSENTIAL JOB FUNCTIONS TREATMENT TEAM PARTICIPATION Participates fully in Clinical Supervision. Coordinates all aspects of team responsibilities. Attends and participates in patient staffing\u2019s. Participates in Performance Improvement activities as assigned by supervisor. Provides effective crisis intervention when required. THERAPEUTIC FACILITATION Conducts or facilitates weekly group and family therapy meetings in the form of process groups, didactic groups. Conducts individual therapy when clinically indicated. Holds regular update meetings with families. Provides patient and family lectures regarding chemical dependency, behavioral health, relationship issues. DEVELOPMENT Demonstrates evidence of continued development within field. Identifies own learning needs and selects opportunities for self-growth and career advancement. Attends in \u2013 service, continuing education, and resolving problems. Uses initiative in identifying, communicating and resolving problems. Other duties as directed by supervisor and as needs of facility dictate. Conducts clinical to behavioral health professional at least once in a two week period depending on hours worked. CASE MANAGEMENT Establishes tracking systems. Organizes, reviews, prepares and tracks assessment work. Reviews open patient records to assure compliance with policy and procedures and delivery of services per treatment plan. Develops home contracts, level contracts, goals and objectives. Develops referral sources. REFERRAL Establishes, develops and maintains communication with referral sources. Provides information as needed for other agencies and individuals. DEPARTMENT SPECIFIC DUTIES Clinical position that provides comprehensive care coordination by working collaboratively with patient's treatment team. A primary function of the position is the\u00a0development and implementation of\u00a0discharge planning. Facilitate daily process and\/or psychoeducation group. Clinical Supervision provided to those pursuing independent licensure. QUALIFICATIONS EDUCATION Masters in Social work, Counseling, Psychology or other Behavioral Health or Human Service field. \u00a0 QUALIFICATIONS Required: Masters in social work, counseling, psychology or other behavioral health or human service field required. Must provide documented competency of one (1) year experience in general counseling, treatment plan development, chemical dependency assessment and group counseling experience. Knowledge of the concepts and principles of chemical dependency and behavioral health related issues. Clinical Supervision provided to those pursuing independent licensure. CERTIFICATION\/LICENSES Have or be eligible for LMSW licensure within 90 days of hire.\u00a0 Have or be eligible to receive the AZ, DPS Fingerprint Clearance. CPR\/First Aid and CPI training or the ability to obtain at hire.\u00a0 EXPERIENCE Coordinates and participates fully in program planning and development. Plans and coordinates effective treatment interventions. Conducts initial evaluation procedures. Evaluates clients for appropriateness of placement. Develops comprehensive treatment plans. Develops and maintains treatment goals and objectives. Designs and implements behavioral intervention for patients. Develops and completes all required patient documentation in a timely and accurate manner. Develops and implements discharge planning.
