Position Summary The Care Coordination Assistant works as part of a team assisting the Care Coordinator with administrative tasks such as completing required documentation and forms, coordinating referrals, and obtaining necessary resources. This role requires excellent collaboration, organization, strong customer services skills, and the ability to work\u00a0independently.\u00a0This\u00a0position offers a flexible\u00a0schedule with\u00a0the opportunity to work from home after training in the office for 3 months. Essential Job Functions: Responsible for the required activities related to Care Coordination assessment, care planning, and coordinating services Locate resources and services such as medical equipment, social services, and county resources Arrange referrals Complete required documentation and forms Telephone customer service, responding to member needs Scheduling appointments Assisting members throughout hospitalization and discharge Outreach telephone calls to members Must have a valid driver\u2019s license; 10% local travel is required \u200b\u200b\u00a0 Qualifications: \u00a0 High school diploma required. Associate\u2019s degree in Human Services, Public Health Community Service, or Health Care Administration preferred. Knowledge\/Skills\/Abilities: Understanding of community resources Knowledge of health plan benefits and Medical Assistance preferred Familiar with medical terminology preferred Comfortable navigating electronic medical record systems and working with patient care notes Detailed oriented and accurate Excellent interpersonal and customer service skills Ability to problem solve and meet deadlines Computer proficient Demonstrated ability to read, write, speak, and understand the English language, bilingual candidate a plus Demonstrated compatibility with Bluestone\u2019s mission and operating philosophies Working Conditions Work Environment -This job is primarily office based with occasional local travel if necessary. This role routinely uses standard office equipment such as computers, phones, photocopiers, file cabinets, and fax machines. Physical Demands - The employee is regularly required to remain a stationary position for long periods of time or occasionally move about the inside the office to access file cabinets and office machinery. The position would require the ability to lift or carry office equipment such as computers and file folders.\u00a0 Travel \u2013 Must have a valid driver\u2019s license; 10% local travel is required Why Bluestone?
