This is a new grant-funded 40-hour (1.0 FTE) Ambulatory Social Work Department position in the Infectious Disease HIV\/AIDS Treatment Program. The position will require the ability to identify and assess psychosocial needs and provide intensive medical case management to adult patients with HIV\/AIDS around issues unique to this population as well as to their families and supports. Professional experience related to illness and hospitalization, mental health and substance abuse, strengthening coping skills, and knowledge of available community resources are integral components of this position. Expertise with biopsychosocial issues, service delivery, and treatment strategies with patients with HIV\/AIDS is required. Duties include identifying patients who require outreach and intensive case management services. Interaction with clinic staff, chart review, and referral from other UMHS departments will be involved in identifying outreach patients. Patient work will include identifying and overcoming barriers to medical care by thorough assessment of mental illness, substance abuse, insurance concerns, lack of transportation, childcare needs, etc. This position includes provision of psycho-education, supportive individual and\/or group counseling, and assistance with identifying specific issues that adversely impact each patient\u2019s health care. This position is grant funded by the State of Michigan through September 30, 2019, but is very likely to continue to be funded through the Ryan White Part B funding as allocated by the Michigan Department of Community Health. Responsibilities* This may include, but is not limited to: Crisis intervention with patients\/families\/supports; psychotherapy and education with patients\/families\/supports who are experiencing difficulty with adjustment, coping, anxiety, depression, and grief issues; and with non-adherent patients with the goal of achieving desired treatment outcomes and enlisting improved or full treatment adherence; bereavement services to patients and families\/supports of patients who are imminently dying or experiencing complicated grief; provision of risk reduction education and counseling; and information and education on Durable Power of Attorney and Advance Directives as requested; advocacy as needed on behalf of patients\/families\/supports to maximize access to appropriate and necessary services to enhance health care; and working closely with non-medical case managers and clinical staff to effectively coordinate care. Required Qualifications* Qualified candidates must have a Master\u2019s Degree in Social Work (MSW) from a school accredited by the Council of Social Work education as well as current clinical licensure to practice social work in the State of Michigan. Excellent written and verbal communication skills, experience with crisis intervention, excellent psychosocial assessment skills, the ability to establish therapeutic relationships quickly and to relate well to diverse patients and their families is required. Proficiency in maintaining accurate and timely clinical documentation using electronic health records, exercise of independent decision-making, team meeting participation, excellent organizational skills, and the ability to effectively prioritize multiple competing tasks are necessary. This position requires flexibility, accessibility within the clinic, the ability to work effectively with a multidisciplinary team, and the ability to work independently in a variety of demanding situations. Desired Qualifications* The ideal candidate will have a strong mental health background as well as medical social work experience. The ability to work in a fast-paced environment, manage a large caseload with multiple competing demands, and knowledge of local, regional and national resources is desirable. Familiarity with networks of HIV and AIDS-related services and organizations, as well as with administrative requirements of the Ryan White Care Act and other grants is desirable. Work Schedule This is a 40-hour (1.0 FTE) position with weekday hours from 8:00-4:30. Some flexibility may be required. Background Screening Under Michigan law, a criminal history check including fingerprinting is required as a condition of transfer or employment for this position.
