Responsibilities The incumbent of this position will complete and oversee a variety of assignments at the discretion of the Chief of Social Work and be responsible for collecting information from clients, families and community agencies. This individual will serve as the Hospital Access Coordinator for East Central Regional Hospital and coordinate with staff in the development, implementation and evaluation of individual treatment plans. This position will gather information in regards to the specific services available to interview, assess and represent clients in order to meet their specific needs; screening, developing, responsible for the supervision and management of social work services in the admission and temporary observation unit; monitoring and coordinating placement plans will be a major responsibility of this position.The duties include, but are not limited to:\u00a0ensuring that services and placement plans meet the client's needs and expectations; serve as a key advisor to a program director and develop strategies and objectives to accomplish the goals of the client as well as the facility; member of a care team in order to coordinate and integrate with the community support programs; experience with policies and procedures pertaining to the care and placement of the client; serve as a liaison for clinical departments or between hospital and community support programs; ensure objectives and priorities for patients are in line with the department and in support of overall hospital goals; able to contribute to a high performance environment characterized by positive leadership and strong teamwork; experience with risk assessment and understands cost-benefit and the impact of placement options, understands regulatory policies and procedures such as those outlined by the Joint Commission and HIPAA and performs other duties as assigned.
