Reports to the Health Centers Manager. Under general

direction, is responsible for coordinating access to medical and social

services and community resources for the patient/client and provides

appropriate care coordination to patient/client. Communicates directly

with the health care provider on all clinical directives. Provides social

assessment, care management, client advocacy and referral services;

collects intake information and documentation to facilitate client

enrollment into desired services. Promotes health education and

participates in various health screening activities, education, and

patient/family group visits. This position involves community outreach and

home visits.

JOB SPECIFICATIONS

Education and Experience

The knowledge, skills and abilities as indicated below

are normally acquired through the successful completion of a Bachelor’s

degree in an allied health or human services field. Minimum of two years

of professional experience in a health or human services setting where

case management skills were utilized is required. The ability to speak

and write Spanish is desired, but not required.

Knowledge & Skills

1.     Demonstrates basic knowledge of family communication skills,

community agencies, services and resources available to clients and their

families.

2.     Requires a solid understanding of medical terminology and

demonstrates knowledge of Health Educator topics such as Diabetes, Heart

Disease, Hypertension, etc.

3.     Requires well-developed analytical, organization and

problem-solving skills. Also demonstrates the ability to adjust to the

different cultures within the community and act appropriately to better serve

our patient populations.

4.     Demonstrates the interpersonal and communication skills

(both verbal and written) necessary for effective interpersonal interaction

with clients, families, physicians, Hospital staff (at all levels) and a

diverse general public. Also must be able to communicate effectively regarding

sensitive issues.

5.     Requires self-motivation, good organization and time

management skills.

6.     Requires the ability to establish and maintain effective

working relationships with clients/patients, families, staff, medical providers

and the public.

7.     Requires good listening and feedback skills, as well as the

ability to accurately assess difficult situations and respond accordingly.

8.     Requires the ability to use good judgment and maintain one’s

composure in any stressful situations.

9.     Demonstrates proficiency in basic computer skills (i.e.,

data entry, word processing and spreadsheets). A basic understanding of

database applications is desired.

Identifies and enrolls

individuals who are in need of healthcare services and/or community resources

by:

individual and family needs including health and social services; providing

education about services to clients and their families.

Assisting clients in accessing

healthcare services.

Acting as an advocate to families.

Linking clients with all needed

services; connecting clients to support groups that would provide emotional and

educational support for patient and their family.

Developing and maintaining community

resource materials and contacts.

Helping clients identify a personal

support system.

2.     Provides care coordination for clients and maintains accurate

records of services provided by:

Providing case management as

necessary to ensure client goals are met, i.e. assist clients/patient with

improving their health risk behaviors as identified by appropriate staff.

Maintaining client/patient contact,

including home visits, as necessary to provide support, encouragement and

guidance.

Working with provider and

multi-disciplinary teams to develop and implement an individualized

intervention / action plan for patient/client and families and assure action

plans are carried out.

Following up on referrals made to

families and make referrals to other appropriate services and resources to meet

the needs of the patient/family.

Providing individualized educational

and emotional support in accordance with the intervention plan.

·        

Collecting data, maintaining

records, reports, statistics and files as required by departmental policies and

procedures; also keeping accurate records of home visits, appointments and

referrals.

Preparing reports and other

documentation as required.

·        

Following up on clients who have

missed an appointment and/or referrals.

3.     Conducts and promotes health education, care management and support

groups, in a language/culturally-appropriate manner within the clinics and/or

community, by:

Creating, conducting and evaluating language/culturally-appropriate

health education programs and support groups based upon identified needs (i.e.

Diabetes, Heart Disease, Asthma, Hypertension, etc.).

Maintaining a key role in the

Chronic Disease Management Model/Team.

outreach activities for the community whenever possible.

Participating in various health

screening activities in the community.

Maintaining effective relationships

with schools, other health systems, healthcare providers, agencies, grassroots
