General Summary:
Assists social work staff in the provision of concrete services to renal patients.
Identifies needs of the patient and family and makes referrals to appropriate community agencies and hospital departments.
Other duties include maintenance of records and statistical data and the provision of clerical support to the social work staff.
Performs related duties as assigned.
Demonstrates team member standards and adheres to diversity principles and expectations including deaf and hard of hearing guidelines.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Assists patients/families with referrals to community agencies and/or other hospital departments for such concerns as transportation arrangements, outpatient nursing home placements, billing/insurance inquiries and utilities.
Acts as a liaison between patients and various community agencies/hospital departments.
Maintains and updates files on available resources.
May assist in the determination and arrangement of emergency appointments for patients who are in crisis situations.
Maintains social work records in accordance with established guidelines.
Assists in the compilation of statistical information concerning patient population and/or social work functions as appropriate.
Provides clerical support to social work staff as necessary (i.e., filing, xeroxing).
Records own activities as necessary in order to ensure accountability.
Travel to satellite units as required.
Demonstrate willingness and ability to function consistently as a team
member, positively contributing on a daily basis to the goals and
of the dialysis program as appropriate to the scope of position.
Demonstrate willingness and ability to work effectively in a complex
and changing environment.
Demonstrate willingness and ability to effectively relate in an organizationally acceptable manner to team members, patients, and other customers in a diverse environment.
Demonstrate a caring and compassionate manner toward the patients and their family members that are a part of the dialysis program.
Complete all required and assigned in-services and online education and
demonstrate incorporation of the teaching/learning as appropriate to position.
EDUCATION/EXPERIENCE REQUIRED:
Associate's Degree or equivalent experience.
Minimum of one year experience in health care environment.
Ability to analyze pertinent patient information in order to refer patients to appropriate agencies/departments.
Interpersonal skills necessary to work with a wide range of patients, families, multidisciplinary hospital staff and individuals from community agencies.
Organizational skills required to manage a high-volume workload.
