The program is designed to meet the needs of Kaiser members who are high users of inpatient psychiatric services or who are at high risk of acute psychiatric hospitalizations. Our teams work closely with Kaiseru0026rsquo;s Psychiatrists and Psychologists to ensure best of class treatment and support services. u0026nbsp;All services are provided in the community.u003c/pu003enu003cpu003eOur inter-professional team partners with Kaiser Permanente psychiatrists and clinical staff to provide community-based, integrative therapeutic skills building, crisis intervention, short-term case management services and support navigating the mental health system.u0026nbsp; Eighty percent (80%) of all services are provided in the community where our members reside.u0026nbsp; We serve 195 Kaiser members- aged 18 , in the Sacramento and upper central valley communities.u0026nbsp; Our geographical area covers 9 Kaiser medical centers, 60-mile radius travel required.
