Job Description Position Summary: The Health Home Criminal Justice Manager will be responsible for collaborating with the Health Home, the State, Correctional Health Services and Rikeru2019s Island to connect individuals released from incarceration to community-based care coordination services.u00a0 Duties and Responsibilities: Traveling to Correctional Health Services offices in Elmhurst, Queens (frequency to be determined) to review and process a list of individuals currently incarcerated to determine affiliations between the individual and the Health Home. Responsible for sharing data reconciliation with Correctional Health Services and necessary follow up. Working with Correctional Health Services discharge planners to communicate the status and next steps of the referrals. Communication with Care Management Agencies regarding possible Health Home referrals or the status of members that may be incarcerated. Partner with Correctional Health and Care Management Agencies to arrange care conferences for individuals being referred to care coordination. Tracking and submission of criminal justice initiative metrics/data and reporting. Participate in State and Correctional Health Services calls and meetings. Support the AVP, Health Home and Director, Health Home Operations on additional criminal justice deliverables. Work closely with the Director, Health Home Quality to conduct chart reviews and monitor quality improvement and performance. Qualifications Certificate/License Required:u00a0 Bachelor's degree. 2- 4 years experience preferably in public health, human services, criminal justice, sociology or related field. Excellent interpersonal, communication, and presentation skills. Excellent written communication skills. Ability to work independently with minimal supervision. Ability to handle complex and confidential information (e.g. State and protected health information) with discretion.
