The Homeless Services Support Staff/PATH Outreach Worker is responsible for providing outreach services to contact and engage people not currently connected to mainstream services. Eligible services through the PATH program are as follows: Outreach, habilitation and rehabilitation, referrals to community mental health, alcohol and drug treatment, and housing resources. Additional job duties will include case management, transportation and assistance to disabled clients with mental and/or physical disabilities in residential setting with activities of daily living, medical appointments, vocational/educational activities, housing search and stability. The overall goal of the program is to provide permanent housing with support to the disabled chronically homeless. In addition, the Homeless Services Support Staff/PATH Outreach Worker provides social support, skills training, and assistance with independent living. The Homeless Services Support Staff/PATH Outreach Worker will uphold and promote the program philosophy in all duties performed.
Hours
Full time position 40 hours per week.
Experience and Skills:
Bachelor’s Degree in a related Human Services field and 6 months related experience
or
at least three years related experience. Conditions of employment include: Ability to possess and maintain good physical and mental health. Must be at least 18 years of age, possess a valid CA Driver’s License, at least 2 years driving experience, reliable transportation, proof of auto insurance, and DMV driving record that meets TMHA’s current vehicle insurance requirements. May be required to utilize personal vehicle and auto insurance. Attendance is an essential function of this job that requires face-to-face interaction with clients and/or other employees. Must comply with a TB assessment (possibly a TB test if needed to confirm a negative tuberculosis result) and receipt of a criminal record clearance through fingerprinting Must provide proof of COVID-19 vaccination, including booster, or request a medical exemption with a healthcare provider document advising against the vaccine OR an exemption based on a strongly held religious belief or practice per the California Department of Public Health. Those who request and are approved for an exemption will be required to test for COVID-19 weekly and send the results to TMHA’s Human Resources Director every 7 days. Must be able to successfully complete First Aid and CPR training.
