Description

The Community Support Specialist, in collaboration with the team, provides community support services to deaf, deaf-blind and hard of hearing individuals, including those who have other disabilities. Responsibilities include the identification and provision of community based support and education services in accordance with consumer’s individualized service plan. In addition to these services, this position will provide outreach and education services to the community at large, including but not limited to presentations about health, self-advocacy, employment and cultural sensitivity trainings to businesses that employ deaf and hard of hearing individuals.

Here are some of things that you'll get to do:

Provides community support services (e.g. independent living skills training, advocacy, benefits counseling, socialization and recreational activities and employment support)

Provides technical support for iPad devices distributed through the Telecommunications Equipment Distribution (TED) program.

Maintains confidential service records within the organization’s database and files to comply with programmatic requirements

Networks and work collaboratively with other service providers (e.g. case managers, mental health providers, and civic and nonprofit organizations) in a manner that is beneficial to consumer’s service goals, including community presentations and outreach

Plans and coordinate joint educational and recreational activities to maintain consumers’ community involvement and socialization skills

Devises outreach plans to local community. Solicit feedback from community members on community needs and work with the Director and team for possible solutions

Performs additional responsibilities/tasks as assigned or requested by State Director to support division priorities

Requirements

Bachelor’s degree in human services related field. High school diploma plus work equivalent may be considered

Fluency in American Sign Language and other communication strategies for communicating with the non-signing hearing loss populations

Experience providing direct human services to deaf and hard of hearing consumers
