Medical City Las Colinas, built in 1997, is a 100 bed full service, acute-care facility offering a wide range of services to the community and families we serve. In addition to our exceptional care and facilities, Medical City Las Colinas is fortunate to partner with over 400 highly trained physicians who practice on our campus. Many of these physicians are located in one of the three medical office buildings located directly on campus or in offices nearby.
The Social Worker: * Provides direct social service and counseling to patients/families * Assists in locating appropriate resources for post-discharge care * Participate in multidisciplinary team to coordinate the client’s plan * Cross-trains with team members to serve as back up resource * Assesses and identifies cases for intervention * Prioritizes goals and develops plans based on client needs * Monitors and evaluates outcome of client’s plan * Proactively identifies cases * Collaborates with third party payors internal case management/disease management departments as appropriate for discharge planning needs
Education: * * Bachelor's Degree Required, Master's Degree Preferred
* Bachelor’s or Master’s degree in Social Work * Current licensure by Texas State Board of Social Work and LMSW a plus.
