The Priority List Manger is responsible for facilitating referrals for all supportive housing vacancies for adults within the Suburban Metro-Area Continuum of Care (SMAC). This region includes Anoka, Dakota, Scott, Carver, and Washington counties. Using HMIS, this position will maintain a priority list of households experiencing homelessness within SMAC from most vulnerable to least vulnerable. This position is a part of a larger SMAC Coordinated Entry System (CES), and will collaborate county Points of Access, housing providers, and other CES stakeholders.
This position is funded by the Housing and Urban Development (HUD) Continuum of Care Program (CoCP) grant and is to be renewed on an annual basis. This position is responsible for executing all requirements identified within the HUD contract.
This is a full time regular position with a workweek expectation of 40 hours per week.
\u2022 Collect and organize assessments and supplemental questions from all SMAC Points of Access using HMIS\u2022 Maintains and manages the communication between assessors, HMIS, and housing service providers to facilitate the efficient and effective referral of singles and families experiencing homelessness to the appropriate housing service.\u2022 Manage and maintain the housing priority list using HMIS\u2022 Collect vacancies from housing providers \u2022 Using a client-centered approach, match homeless households on the priority list with the most appropriate supportive housing opening using HMIS\u2022 Build relationships with housing providers and become knowledgeable of program enrollment and eligibly\u2022 Creates and maintains an up to date record of individual program eligibilities in HMIS with which the manager will use to make referrals.\u2022 Responsible for tracking and reporting CES outputs and outcomes to SMAC and to HUD\u2022 Provide coverage to Priority List Manager counterpart during times of absence.\u2022 Work in collaboration with Youth Priority List Manager employed by Lutheran Social Services (LSS) StreetWorks Collaborative\u2022 Process client and provider grievances and discuss options for resolution (as needed)\u2022 Assist Twin Cities Social Services Assistant Director in SMAC Notice of Funds Available (NOFA) annually to renew funding\u2022 Actively participate in the SMAC CES committee to help build and maintain relevant policies \u2022 Attend local SMAC regional meetings as needed\u2022 Represent the program in the professional community and maintain working relationships with community resource personnel; advocate for the program and work to generate and maintain referral sources\u2022 Maintain positive work atmosphere by behaving and communicating in a manner that fosters and maintains positive relationships with co-workers and supervisors. This would include but not be limited to such actions: resolution of conflicts in a positive manner; courteous treatment of staff, clients and visitors; respect of others\u2019 property and person, professional and appropriate communication to and about co-workers, supervisors and subordinates. \u2022 Bachelor\u2019s degree in a human services or related field\u2022 Experience working with Minnesota\u2019s Homeless Management Information System (HMIS) preferred\u2022 Two to Three years experience in social service delivery for individuals experiencing homelessness\u2022 Demonstrated working knowledge of supportive services and other resources for individuals experiencing homelessness\u2022 Demonstrated ability to advocate, organize, problem-solve and provide results for clients and co-workers\u2022 Ability to accept supervision and work with a team of co-workers\u2022 Must have the ability to operate a computer, use a keyboard to enter information and enter client information into HMIS database.\u2022 Must be able to work independently\u2022 Willingness to support the philosophy, goals and mission of The Salvation Army.\u2022 Must pass the background checks\u2022 It must be understood by the incumbent that the provisions of Safe From Harm will apply upon commencement of employment and also understood that the required Safe From Harm training be satisfactorily completed within the first thirty (30) days of employment.
