(BSHC) that assists low-income residents who reside in elderly/disabled housing complexes to age in place and make lifestyle changes that improve the quality of their life. The Community Resource Specialist (CRS) assists residents in identifying needs to stay in place and independent and make appropriate community referrals. The CRS works closely with the building's Resident Services Coordinator and MGH Registered Nurse (RN) to identify residents who might be in need of Home Care or other services, and provides assistance with tenancy issues, as appropriate, to help avoid eviction. The CRS will coordinate any needed ongoing case management to residents who are eligible for services from **MEMBERS ONLY**SIGN UP NOW***.. nnThe CRS must possess advanced skills in consumer assessment and be able to assess the physical conditions of the consumer's home as well as the consumer. (Interpersonal flexibility, initiative, teamwork, a customer service attitude and excellent oral and written communication skills are key qualifications for this position as are solid organizational skills)nnThe CRS will work in partnership with the MGH RN and BSHC contracted Clinical Social Worker (SW) assigned to the three identified Housing sites. The team will create and follow communication pathways to identify and notify any change in status for assigned residents who may require an intervention in order to avoid hospitalization or nnThe incumbent is responsible for both identifying issues that arise in a building and effectively working with all partners. The CRS will work with ASAP and non-ASAP consumers and any resident regardless of age. nEssential FunctionsnEssential Job functions: Duties ____1___ through ___11____are designated as ADA Essential Functions and must be performed in this job. All other job duties are secondary functions. [TBD]nnServe as a resource person for the assigned Supportive Housing building by providing knowledge on consumer and government resources etc. to both ASAP and non-ASAP residents.nCreate a schedule in conjunction with RN and SW to be on site as needs unfold. Presence at each site would be a maximum of seven hours per assigned day. nWork in concert with MGH RN and SW to identify residents who seek or require assistance. nHave access to MGH database to be able to communicate resident issues, progress and journal entries to share with MGH RN. CRS will generate or request reports on residents in said buildings every 30 days or no fewer than quarterly. nDevelop and maintain positive relationships with Supportive Housing partners nTo be trained on Evidence Based classes and conduct trainings to the residents.nAssist Program Coordinator in developing and implementing outreach activities to increase enrollment for all BSHC care management programs. nFoster a sense of community by having residents be tolerant and respectful of other residents and their property. nPlan and direct health and wellness workshops and educational programs. No less than 2 per building over a twelve month rolling period, 6-8 week Evidence Based Trainings (Examples include Tai Chi, Matter of Balance, Chronic Disease Self-Management) Develop and implement procedures for engaging consumers in planning, implementing, leading and participating in activities. nLeverage and outreach to community partners for topical and educational presentations to residents. nPerform other duties as assigned by Team Manager.nnCompetenciesnnMust possess advanced skills in consumer assessment and be able to assess the physical conditions of the consumer's home as well as the consumer. nInterpersonal flexibility, initiative, teamwork, a customer service attitudenExcellent oral and written communication skills are key qualifications for this position nSolid organizational skillsnnSupervisory ResponsibilitynThis position has no supervisory responsibilities.nWork EnvironmentnThis job operates in a professional office environment.
