Summary
Assigned 75% to the Community Living Center and 25% to the Adult Day Health Care Program. Is responsible for providing social work services to Veterans. Is a member of an Interdisciplinary Treatment Team (ITT). Is primarily responsible for completing biopsychosocial history and assessments, completing psychosocial portions of care plans & Minimum Data Set (MDS). Is responsible for facilitating ITT meetings, emphasizing culture transformation, including resident centered care planning.

Responsibilities
The incumbent is responsible for completing the following duties/responsibilities:
Provide orientation to new residents of the CLC/ADHC, communicating with Veterans & family members/significant others the purpose of CLC, expected length of stay & goals for discharge on admission
Conduct biopsychosocial assessments to determine needs of Veterans & family members
Participate in the treatment planning process with other disciplines
Facilitate the Long Term Care Co-Pay process with the Veteran & family members
Participate in discharge planning with other disciplines, Veterans & family members. Will be responsible for ensuring discharge plans are executed timely and appropriate
Provide consultation/education to Veterans & families on community resources & VA services/benefits
Act as a consultant to other team/staff members
Act as liaison between VA & community resources
Provide case management to Veterans & families, as needed, throughout the continuum of care.
Provide a full range of social work services within commonly accepted standards of social work practice
Provide psychosocial treatment to assist Veterans & family members with psychosocial concerns
Assist Veterans with crisis situations
Serve as the CLC & ADHC contact person for family members/significant others. Will coordinate family conferences/meetings & address family issues that impact the Veteran's health/functioning
Serve as an advocate for residents with facility staff & treatment team members
Assist Veterans address advance care planning and end-of-life issues
Provide wellness/prevention education & facilitate resident/family support groups as appropriate
Supervise the practice of social work students, or a less experienced social worker as applicable
Coordinate treatment & communicate with the care lines to facilitate continuity of care for veterans
Participate in various Service Line/health care system workgroups, committees, and/or councils to represent and contribute social work expertise as assigned
Maintain data & statistical compilations to comply with Joint Commission, VA and health care system policy and
procedures
Document all social work interventions and activities in the resident's clinical record utilizing CPRS and ensure appropriate hand off when transferring patient care
Work Schedule: Administrative Days, Monday-Friday, 8:00 a.m. to 4:30 p.m

Qualifications
Basic Requirements
:
* U.S. Citizenship.
Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
* Education.
Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
* Licensure.
Licensed or certified by a state to independently practice social work at the master's degree level. EXCEPTION: VHA may waive the licensure or certification requirement for persons who are otherwise qualified, pending completion of state prerequisites for licensure/certification examinations. This exception only applies up to the full performance level. For grade levels above the full performance level, the candidate must be licensed or certified.
* Physical Requirements
. Must be able to pass a physical if required.
* English Language Proficiency.
Social workers must be proficient in spoken and written English.
Grade Determinations:
GS-9
is the entry level grade for the GS-185 social work series and is used for social workers with less than one year of post-MSW experience and for social workers who are not yet licensed or certified at the independent practice level. Social workers at the GS-9 level are working toward completion of prerequisites for licensure or certification.
NOTE: Social Workers with more than one year of post-MSW experience may be appointed at an advanced step within the GS-9 grade commensurate with their years of experience.
GS 11
Full performance level requires completion of a minimum of 1 year of post-MSW degree experience in the field of health care social work and licensure or certification in a state at the independent practice level.
NOTE: For appointment licensure or certification at this level please refer to paragraph 2c.
OR,
In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
Grade Determinations
: Salary to be determined by Professional Standard Board of your peers and based on your education, experience, and professional accomplishments.
The full performance level of this vacancy is GS-11. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-9 to GS-11.
